The short version: yes, digoxin reintroduction is documented to improve heart failure, and the improvement can begin within hours, but the trials that prove it are old, small, and mostly industry-adjacent.
The strongest evidence comes from the withdrawal trials themselves. In PROVED and RADIANCE, patients who stopped digoxin worsened; the implication is that restarting it reverses that worsening [1]. The 1987 study gave intravenous digoxin to patients already on diuretics and vasodilators and showed further improvement in cardiac function [4]. The 1980 study using systolic time intervals showed oral digoxin begins working in about 2 hours and peaks at about 8 hours; intravenous forms act faster, within minutes [2]. So the mechanism is real and the time course is hours to days, not weeks.
The 2015 study adds a caution: patients who stopped digoxin had worse BNP and walk distance at 12 weeks, but reported better quality-of-life scores [3]. That is a strange finding, and it suggests the subjective experience of being on digoxin is not uniformly positive. The 2015 BMJ meta-analysis is the most important check on enthusiasm: in randomized trials, digoxin had a neutral effect on mortality, though it did reduce hospital admissions [5].
