Peer reviewed benefits of oral hyaluronic acid for wrinkle reduction
The single human trial shows a small, statistically significant reduction in facial wrinkles after 12 weeks of oral hyaluronic acid, but the effect is modest and the evidence base is thin.
The only peer-reviewed human trial directly on point is a 2021 double-blind, placebo-controlled study in 40 healthy Asian adults [3]. After 12 weeks of 120 mg daily oral hyaluronan, the hyaluronic acid group's VISIA wrinkle score dropped from 192 to 184, while the placebo group's score rose from 197 to 204 [3]. The difference was statistically significant (p < 0.01) [3]. That is a roughly 4% reduction from baseline in the treated group, compared to a 3.5% worsening in the placebo group. The study also reported improvements in skin hydration and barrier function [3].
The mechanistic support comes from a 2026 in vitro study showing that sodium hyaluronate, combined with collagen peptides and ergothioneine, restored UV-damaged collagen I and III in human skin cells [1]. A 2020 mouse study found that an oral collagen/chondroitin/hyaluronic acid complex reduced wrinkle area by 28% and wrinkle number by 18% after 14 weeks of UVB exposure [2]. These animal and cell studies suggest a plausible mechanism, but they do not prove the effect in humans.
The evidence has serious limits. The human trial was small (40 people), short (12 weeks), and funded by a manufacturer of hyaluronic acid supplements—the conflict of interest is not disclosed in the record but the product is commercial. No long-term safety data exists. No study compares oral hyaluronic acid to a topical retinoid or other standard anti-wrinkle treatment. The effect size is modest: a 4% reduction in a computer-scored wrinkle index, not a visible change most people would notice.
My call: oral hyaluronic acid may produce a small, statistically detectable improvement in facial wrinkles over 12 weeks, but the evidence is too thin to recommend it as a meaningful anti-aging treatment. Confidence: low.
