Is the pneumococcal vaccine effective?

The pneumococcal vaccine works for some people against some outcomes, but the evidence is uneven and the benefit is often smaller than advertised.

The strongest evidence comes from a double-blind, placebo-controlled trial in 1,006 Japanese nursing home residents. The 23-valent polysaccharide vaccine (PPV23) cut all-cause pneumonia from about 21 in 100 to about 13 in 100 over two years, and pneumococcal pneumonia from about 7 in 100 to about 3 in 100 [1]. No one who got the vaccine and still got pneumococcal pneumonia died from it, compared to 13 deaths in the placebo group [1]. That is a real benefit in a frail, high-risk population.

But the picture changes for healthier older adults. A large Finnish cohort of 26,925 people aged 65 and older found that adding PPV23 to the flu shot gave no extra protection against pneumonia or pneumococcal pneumonia [3]. The relative risk for pneumonia was 1.2, meaning the vaccine group actually had slightly more cases [3]. A 2014 South Korean study of 2,262 patients with flu-like illness found the pneumococcal vaccine was "not effective" at preventing pneumonia or hospitalization [2]. A 2021 review of the CAPiTA trial, the largest randomized trial of the 13-valent conjugate vaccine (PCV13) in older adults, found only a small, non-significant reduction in lower respiratory tract infections in primary care [8].

For children, the conjugate vaccines (PCV7, PCV10, PCV13) have reduced invasive disease from the serotypes they target. In Denmark, PCV7 cut invasive pneumococcal disease by 54% in children under 2 [5]. In Finland, PCV10 reduced it by 78% in vaccinated children [7]. But serotype replacement is real: non-vaccine serotypes increase, and the overall decline is often modest [6]. In Taiwan, overall IPD dropped 38% after vaccine introduction, but serotype 19A, not covered by PCV7, rose from 0.5% to 11.5% of cases [4].

The evidence is funded by government grants and public health agencies, not manufacturers, for the key trials [1][3]. But the literature has a structural problem: most studies compare vaccinated to unvaccinated in observational designs, not randomized trials. The one large randomized trial in healthy older adults (CAPiTA) showed modest effects that did not reach significance for most real-world outcomes [8]. The PPV23 meta-analysis from 2024 found no reduction in all-cause mortality and only a reduction in pneumonia-related death [9].

My call: the pneumococcal vaccine reduces serious disease in high-risk groups like nursing home residents and young children, but for healthy adults and older people living independently, the benefit is small and inconsistent. The evidence does not support universal vaccination for everyone over 65. Confidence: moderate.
