Pomegranate supplements

The evidence for pomegranate supplements is thin, conflicted, and mostly from animal or lab studies, not from large human trials that measure real health outcomes.

The only large human trial here tested pomegranate extract against placebo in 183 men with rising PSA after prostate cancer treatment [6]. It found no overall benefit on PSA doubling time or disease progression [6]. A subgroup analysis suggested a possible effect in men with a specific gene variant, but that is a post-hoc finding, not proof [6]. A small 2024 study of 59 people found that a fruit extract blend including pomegranate increased one antioxidant enzyme (catalase) but did not change blood pressure, body measurements, or any inflammatory marker [1]. A 2021 registry study of a turmeric-pomegranate-ginger lozenge reported fewer colds in 26 people, but the control group got a mint candy, not a placebo, and the study was not randomized [7]. That is not reliable evidence.

The rest of the evidence comes from lab dishes and animals. Pomegranate peel extract shows strong antioxidant activity in test tubes [3][4]. In rats, pomegranate extract lowered blood pressure and reduced oxidative stress in the brain [2]. In aging mice, it reduced some markers of oxidative damage in white blood cells [8]. In chickens, pomegranate peel improved feed efficiency and lowered cholesterol but also reduced growth [5]. These are interesting findings, but they do not tell you what happens in a human body over years of daily supplementation.

What is missing matters more than what is here. No long-term human safety data exists in these records. No trial has tested pomegranate supplements against a real placebo for hard outcomes like heart attack, stroke, or death. The antioxidant mechanism sounds plausible, but the human trial that measured blood pressure and inflammation found no effect [1]. The prostate cancer trial found no effect on the disease [6]. The burden of proof has not been met.

My call: pomegranate supplements are unlikely to cause harm at typical doses, but the evidence does not show they provide meaningful health benefits in humans. Confidence: low.
