Is post-vaccine myocarditis truly mild and transient, or can cardiac MRI show lasting scarring?
Cardiac MRI can show lasting scar after post-vaccine myocarditis, even when the patient feels fine and lab tests have returned to normal.
The evidence comes from case reports that followed patients with cardiac MRI over time. A 16-year-old boy who developed myocarditis two days after his third Pfizer dose had troponin levels over 31,000 ng/L (normal below 19) and clear MRI evidence of active myocarditis [1]. His symptoms and lab values resolved within days. But follow-up MRI at 3 months and again at 9 months still showed residual late gadolinium enhancement in the same heart wall segments, without active inflammation [1]. That pattern — gadolinium pooling without edema — is consistent with fibrosis, or scar tissue.
A separate case of a 22-year-old man after his second Moderna dose showed the same thing: cardiac MRI found myocardial edema, hyperemia, and scar in the inferolateral wall [2]. His troponin normalized after treatment with colchicine and ibuprofen, but the MRI scar remained. A study of five adolescents with Pfizer-related myocarditis reported that late gadolinium changes persisted on follow-up MRI, described as "persistent myocardial fibrosis despite resolution of acute inflammation" [3].

These are individual cases, not population statistics. They cannot tell you how often scarring happens. But they prove it can happen. The claim that post-vaccine myocarditis is always "mild and transient" is false in at least these patients. Mild symptoms and transient lab markers do not mean the heart is unscarred. The MRI sees what blood tests and symptom checklists miss.
My call: cardiac MRI can show lasting myocardial scar after mRNA vaccine myocarditis, even after clinical recovery. Confidence: moderate — the mechanism is documented in multiple case reports, but no large systematic MRI follow-up study exists to give a rate.
