The short version: no study has ever tested whether a tetanus booster is safe or necessary in someone with a history of Lyme disease. The question has not been studied.

The evidence you retrieved contains nothing about tetanus vaccination after Lyme disease. The closest records are about tick biology and Lyme vaccine development in mice [28, 30, 32], not about giving tetanus toxoid to a person who has had Lyme. One study on tick salivary proteins [28] and one on engineered mouse antibodies [30] are entirely off-topic. The rest of the evidence covers tetanus immunity in general populations [1, 2, 6, 8, 18], not in people with a prior Borrelia infection.

The mechanism question is real but unanswered. Lyme disease is caused by Borrelia burgdorferi, a spirochete that can persist in tissues and provoke immune dysregulation, including autoantibody formation and chronic inflammation [34]. Tetanus toxoid is a potent adjuvant-driven antigen that pushes the immune system toward a Th2 response. In someone whose immune system has already been derailed by a persistent pathogen, adding a strong adjuvant could in theory worsen immune imbalance, trigger molecular mimicry, or reactivate latent inflammation. But that is a theoretical concern, not a documented one. No study in this retrieval tested it.

My call: there is no evidence to recommend or to warn against a tetanus booster after Lyme disease. The decision rests entirely on the wound risk and the person's tetanus immunization history, with the Lyme history adding an unknown but plausible immunologic variable. Confidence: not clear — the question has simply never been studied.
