What are the risks of consuming excess folic acid?

The short version: excess folic acid is linked to cancer promotion, metabolic harm, and disrupted brain development in offspring, with the strongest evidence coming from animal studies and observational human data.

The evidence here is a mix of animal feeding studies, human observational cohorts, and in vitro cancer cell work. No manufacturer funded these studies; they come from academic and public health researchers. But the burden of proof is on the intervention, and the human data is observational, not from randomized trials. A 2012 review argued that cancer-risk concerns are not supported at physiological intake levels [2], but a meta-analysis of 10 randomized trials found a borderline significant increase in overall cancer with folic acid supplementation, especially prostate cancer [2]. A 2021 review listed masking B12 deficiency, increased cardiovascular mortality, and potential immune or cancer risk from unmetabolized folic acid as established concerns [6].

The animal data is striking. In rats, a 5-fold excess of folic acid during pregnancy altered 646 genes and 910 DNA methylation regions in the newborn brain, affecting protein translation, mitochondrial function, and glutathione pathways [4]. The effect was female-specific. Another rat study found that perigestational high-dose folic acid programmed long-term metabolic dysfunction in offspring: increased weight gain, hyperglycemia, and glucose intolerance, with females more vulnerable [8]. In adult rats on a high-fat diet, excess folic acid increased weight gain by 14%, worsened glucose tolerance, and drove adipose inflammation [3].

The human cohort study from Australia is the most directly relevant: after mandatory folic acid fortification, median red cell folate nearly tripled, 57.6% of pregnant women had elevated levels, and gestational diabetes rose from 5.0% to 15.2% [7]. The risk was dose-dependent: women in the highest folate category had an 18.5% GDM rate versus 12.5% in the normal range. This is observational, so confounding is possible, but the dose-response gradient strengthens the case.

My call: the evidence is strong enough to warrant caution, especially for pregnant women and women of childbearing age who are already getting folic acid from fortified foods. The upper tolerable limit of 1000 mcg per day is routinely exceeded by supplement users in fortified countries [13]. For most people, getting folate from food sources is safer than high-dose synthetic folic acid. Confidence: moderate.
