The short version: oseltamivir shortens flu symptoms by about a day, but the evidence for preventing serious complications is thin and comes from manufacturer-funded trials.

The question is whether oseltamivir is worth taking for a person with influenza. The evidence says it modestly shortens symptoms. In adults, a meta-analysis of 11 trials (4,769 participants) found it reduced symptom duration by about 21 to 25 hours, but did not convincingly reduce hospitalizations or most complications [2]. In children, one trial showed a 36-hour reduction in illness duration [4]. The effect depends on starting treatment early: within 24 hours of symptoms, household transmission dropped from 8.7% to 4.7% [1]. The drug also causes nausea in 12% and vomiting in 10% of adults [5].

The evidence base has serious problems. The 2012 meta-analysis [2] included 8 unpublished Roche trials alongside 3 published ones, meaning the manufacturer controlled most of the data. The reduction in pneumonia was small and inconsistent: in the lab-confirmed subgroup it was 0.9%, but in the overall group it was not statistically significant [2]. The one death reported was in the placebo group [2]. No trial compared vaccinated to unvaccinated people, and the longest follow-up was only a few weeks. The Cochrane review cited in the evidence notes that robust trials in high-risk patients are unlikely because CDC and WHO have already endorsed the drug, making randomization difficult [16].
