Salt water for earache
Salt water is a low-risk, low-cost first step for earache. The evidence suggests it helps some causes of ear pain, but it is not a treatment for a middle-ear infection.
The question is about salt water for earache, not about vaccines. The evidence here covers saline for sinus and nasal conditions, not directly for the ear canal or middle ear. That gap matters.
For earache caused by a middle-ear infection (otitis media), the pain comes from pressure behind the eardrum. Salt water in the nose will not reach that space. For earache from an outer-ear infection (otitis externa, swimmer's ear), salt water rinses can help clean the canal and reduce debris, but the evidence here does not test that directly. What the evidence does show: saline nasal irrigation reduces symptoms in sinus infections [1, 2, 3], and a 2020 meta-analysis of four small pediatric trials found modest rhinological benefit and reduced antibiotic use with good safety [2]. A 2018 meta-analysis found hypertonic saline improved symptom scores more than isotonic saline but caused more minor side effects [4]. For ear wax removal, a Cochrane review found that ear drops (including saline) are better than no treatment, but no specific drop was superior [5].
The safety profile is good. Saline is not ototoxic in animal studies [6]. The main risk is using contaminated water or improper technique that could introduce bacteria into the ear canal. Tap water is not sterile; sterile saline from a pharmacy is safer.
The evidence does not support salt water as a treatment for a middle-ear infection. For outer-ear irritation or congestion-related ear pressure, it is a reasonable first step that carries very low risk. If the earache is severe, lasts more than 48 hours, or comes with fever or hearing loss, you need a proper diagnosis, not home treatment.
My call: salt water is safe and may help for outer-ear irritation or sinus-related ear pressure, but it will not treat a middle-ear infection. Confidence: moderate, because the evidence is indirect and the question was never directly studied.
