The short version: losartan works by lowering blood pressure, and most of its side effects are the predictable consequences of that mechanism, not exotic toxicities.

Losartan is an angiotensin receptor blocker. It lowers blood pressure by blocking a hormone that constricts blood vessels. The main risks follow from that: blood pressure that drops too far, especially when standing up, and a rise in potassium. In the RENAAL trial of 1,513 people with diabetic kidney disease, losartan roughly tripled the odds of potassium climbing above 5.0 mmol/L, and about 1 in 10 people on it reached 5.5 or higher [5]. That matters because higher potassium tracked with worse kidney outcomes in the same analysis [5]. The drug also raises bradykinin, a peptide that dilates vessels, which is the mechanism behind the rare but serious side effect of angioedema, swelling of the lips and throat [1, 2]. One case report describes a 74-year-old man on losartan who developed lip swelling after adding naproxen, so NSAIDs deserve caution in combination [1].

The evidence base here is mostly mechanistic and observational. The bradykinin finding comes from a small human study of hypertensive patients [2]. The potassium data comes from a post hoc analysis of a manufacturer-funded trial, which means it was not designed to answer the safety question directly [5]. No long-term safety registry with a true unexposed comparison group appears in this retrieval. What the records do show is that losartan protects kidney function in several animal models and small human studies, reducing proteinuria and fibrosis [7, 11, 13]. One in vitro study found losartan caused no DNA damage in human lymphocytes, unlike some older blood pressure drugs [10]. A rat study raised a specific concern: losartan-induced low blood pressure was linked to memory deficits and tau protein changes in the brain, though this has not been confirmed in humans [15].
