What specific diet changes are best for CKD stage 3 and cholesterol?
The short version: for CKD stage 3 with borderline cholesterol, the best diet shifts are cutting saturated fat, adding soluble fiber, and replacing animal fats with unsaturated oils, but the evidence for kidney safety is indirect.
The evidence here tests diets in people without kidney disease. A 12-month Mediterranean diet in hypercholesterolemic children lowered LDL from 182 to 160 mg/dL and improved artery thickness [4]. A meta-analysis of plant stanols at 2 g/day cut LDL by 12% on top of a low-saturated-fat diet [12]. A large Iranian cohort found DASH-style diets linked to lower total cholesterol and LDL [9]. But none of these studies measured kidney function or safety in CKD stage 3.
The risk is that some high-protein or high-potassium diets can strain damaged kidneys. The evidence here does not test that. What is safe to infer: replacing butter and palm oil with olive or canola oil lowers LDL [18], and adding 10 g/day of fiber cuts coronary heart disease risk by about 8% [32]. Both are low-risk moves for a CKD patient if potassium and phosphorus are watched.

My call: a low-saturated-fat, high-fiber, plant-focused diet is the best bet for cholesterol, but the evidence does not include a single trial that confirms kidney safety in CKD stage 3. Confidence: moderate for lipid benefit, low for renal safety.
