Do statins impact loss of mental capacity

The short version: statins do not cause loss of mental capacity, and the best evidence suggests a small protective effect against dementia, but the data is observational and the benefit is modest.

The question is whether statins cause cognitive decline. The evidence here does not support that. A 2003 review of 60 case reports found patients who reported memory loss on statins, with symptoms reversing after stopping the drug [15]. But case reports cannot prove causation. The randomized trials tell a different story. A 2005 review of existing statin trials found no consistent evidence that statins worsen cognition [11]. A 2025 umbrella review of 68 meta-analyses found moderate-certainty evidence that statins are associated with reduced dementia risk, with effect sizes ranging from a 15% to 38% relative reduction [7]. A 2020 target-trial emulation in the Rotterdam Study found that sustained statin use was associated with a 2.2% absolute risk reduction for dementia over 10 years, though the confidence interval crossed zero (-5.2% to 1.6%) [6]. A 2014 Taiwan cohort found a 22% lower risk of dementia in statin users, with longer use showing stronger protection [2]. But these are all observational. People who take statins are different from people who do not: they see doctors more, manage other risk factors, and may have healthier lifestyles. The randomized trial evidence does not show a clear cognitive benefit.

The mechanism question matters. Statins lower cholesterol, and the brain needs cholesterol for myelin and synaptic function. In theory, aggressive lowering could impair cognition. But the trial data shows this does not happen at clinically relevant rates. The more plausible mechanism for any cognitive benefit is vascular: statins reduce stroke and improve blood flow, and vascular health is tightly linked to dementia risk [18].

My call: statins do not cause loss of mental capacity. The evidence for a protective effect against dementia is suggestive but not proven, and the absolute benefit is small. For someone already on a statin for a clear cardiovascular indication, the cognitive risk is negligible. Confidence: high for no harm, moderate for a small protective effect.
