Question explored with the scientific record
Statins can inrease risk of atrial fib
The short version: in the studies retrieved, statins mostly lowered atrial fibrillation risk, not raised it. The one clear signal of harm came from a different drug class.
The question as asked is about incident atrial fibrillation in ordinary statin users. Almost everything retrieved is about a narrow setting: patients around heart surgery or after a heart attack. In that surgery setting, statins cut post-op AF roughly in half in several analyses [1][6][7], and one randomized trial found no AF benefit at all [5]. In a large Taiwanese cohort of heart-attack survivors, statin use was linked to a slightly lower risk of new AF [2], and continuing statins was linked to lower AF risk in another cohort [3]. These are observational, so they show association, not proof of cause [2][3].
| Setting | Statin group | Control group |
|---|---|---|
| Post cardiac surgery [1] | 17.9% AF | 36.1% AF |
| Post cardiac surgery [7] | 12.1% AF | 23.4% AF |
| Randomized surgery trial [5] | 21.1% AF | 20.5% AF |
| Heart-attack cohort [2] | lower risk (HR 0.935) | reference |
The AF harm signal in this retrieval belongs to high-dose omega-3 fatty acids, not statins: 3.1% vs 2.1% hospitalization for AF in one trial, and a higher rate of new AF in another [4]. That is a real, separate finding.
What this retrieval cannot see: no large trial of statins in healthy people with AF as the measured outcome. The evidence here is mostly surgical and observational, so it cannot settle the general-population question.
My call: on this evidence, statins do not appear to increase atrial fibrillation, and may modestly reduce it in some groups, but the general-population question is not answered here. Confidence: low to moderate.

Statins reduce new-onset atrial fibrillation after acute myocardial infarction
In a retrospective cohort study of 27,553 Taiwanese AMI patients from the NHIRD, statin use was associated with a modestly lower risk of new-onset atrial fibrillation (HR 0.935, 95% CI 0.877-0.998, P=0.0427), and existing or new-onset AF was associated with worse 1-year cardiova…

Continuation of statin therapy and a decreased risk of atrial fibrillation/flutter in patients with and without chronic kidney disease
In a population-based cohort of statin initiators from Taiwan's National Health Insurance Research Database, continuing statin therapy (vs discontinuation) was associated with a reduced risk of new-onset atrial fibrillation/flutter in both CKD and non-CKD patients, with a larger…

Omega-3 Fatty Acids in the Management of Cardiovascular Disease Prevention
Evidence is formulation- and dose-specific: high-dose purified EPA reduced ischemic events in selected statin-treated hypertriglyceridemia, while mixed EPA/DHA regimens have shown inconsistent or neutral cardiovascular benefit.
