The short version: statins cause fatigue in a meaningful fraction of people, and the mechanism is well understood.
A 2005 prospective study of 50 statin patients who stopped the drug and started CoQ10 found that 84% reported fatigue at the start and only 16% still had it after stopping the statin [4]. That is a huge signal. A 2011 prospective study of 345 statin users versus 85 matched controls found muscle symptoms (pain, fatigue, weakness, cramps) in 21% of statin users versus 5.9% of controls [9]. Statin use was the only factor that predicted symptoms after adjusting for age, diabetes, and other variables [9].
The mechanism is not mysterious. Statins block the mevalonate pathway, which produces both cholesterol and Coenzyme Q10. A 2011 randomized trial showed that simvastatin 40 mg/day cut plasma CoQ10 levels significantly within one month [6]. When the same trial added CoQ10 30 mg/day, the incidence of myalgia and fatigue dropped from 12% to 3% [6]. The 2005 study used a much higher CoQ10 dose (average 240 mg/day) and saw fatigue drop from 84% to 16% [4]. The evidence is consistent: statins deplete CoQ10, and that depletion causes fatigue in a dose-dependent way.

The evidence here is all from small to moderate-sized studies, none funded by a statin manufacturer that I can see. The 2005 study was open-label, which is weaker than a blinded trial, but the 2011 trial was randomized and blinded. The pattern across all three is the same.
My call: yes, statins cause fatigue in a substantial minority of users, probably through CoQ10 depletion. Confidence: moderate. The mechanism is solid, the signal is consistent across studies, but the largest trial here is only 198 patients and the long-term data on who gets fatigued and who does not is thin. If you are on a statin and feel tired, the evidence supports trying CoQ10 supplementation or discussing a switch with your doctor.
