Do studies compare vaccinated vs unvaccinated groups for epilepsy risk?

The short version: no study in this retrieval directly compares epilepsy incidence in vaccinated versus unvaccinated people.

The retrieved records all study people who already have epilepsy, asking whether vaccination worsens their seizures. An Italian study of 358 adults found 25 (about 7 in 100) had a transient increase in seizure frequency, mostly among those with poorly controlled epilepsy [1]. A 2024 study of 786 patients found no significant increase in average seizure frequency after vaccination [2]. A 2025 Saudi cross-sectional study of 67 patients found no statistically significant association between vaccination and seizure frequency change [3]. None of these include an unvaccinated control group for comparison.

The question asks about new epilepsy developing after vaccination, not seizure worsening in existing epilepsy. The closest evidence is a 2025 Iranian cohort of nearly 90,000 adults that found 5 cases of generalized seizure after Sputnik V and AZD1222 [5], and a Mexican pediatric surveillance review that reported 2 seizure cases after 1.99 million doses [6]. But a single seizure is not epilepsy, which requires recurrent unprovoked seizures. No study here follows people long enough to make that diagnosis.

A 2023 case report describes a 19-year-old man with a history of epilepsy who died after COVID-19 vaccination and infection, but his death was linked to multiple sclerosis flare, not new epilepsy [4]. A 2022 case report describes anti-NMDA receptor encephalitis after vaccination, which can cause seizures, but this is a rare autoimmune condition, not typical epilepsy [6].

My call: the evidence does not answer whether COVID-19 vaccines cause new epilepsy because no study in this retrieval compares vaccinated to unvaccinated people for that outcome. Confidence: low.
