The short version: the evidence is clear that metformin causes vitamin B12 deficiency in a substantial fraction of users, and the official label understates the risk.

The link between metformin and B12 deficiency is one of the better-documented adverse drug effects in diabetes care. A 2024 meta-analysis found that metformin use is associated with decreased B12 concentrations across exposure durations from 6 weeks to 48 months, with reported deficiency rates ranging from 6.3% to 35% [1]. That same review explicitly states the prevalence is greater than what is declared on the regulatory Summary of Product Characteristics [1]. A 2010 BMJ randomized placebo-controlled trial—one of the strongest designs available—found that after long-term treatment, 9.9% of metformin users developed B12 deficiency versus 2.7% on placebo [9]. A 2014 meta-analysis of 29 studies covering 8,089 patients calculated an odds ratio of 2.45 for B12 deficiency with metformin use [3].

The risk is dose-dependent and time-dependent. A 2026 Ugandan study found that a dose of 2000 mg/day carried an adjusted odds ratio of 4.63 for deficiency, and use for 10 years or more carried an aOR of 4.04 [5]. A 2016 Indian study showed mean B12 levels dropping from 183 pg/mL in those taking under 1500 mg to 118 pg/mL in those taking 1500 mg or more, and from 163 pg/mL in those using metformin under 5 years to 101 pg/mL in those using it 5 years or longer [6]. A Ghanaian cross-sectional study found a combined B12 deficiency prevalence of 14.3% using both serum B12 and methylmalonic acid, with longer duration and higher dose as significant risk factors [4].

The mechanism is not speculative. A 1983 study directly demonstrated malabsorption of B12 during metformin therapy, with absorption improving after drug withdrawal [8]. The deficiency is not just a lab finding. The 2026 Ugandan study linked B12 deficiency to higher rates of peripheral neuropathy and neuropsychiatric symptoms [5]. A 2007 case report documented a 65-year-old woman on long-term metformin who developed deep vein thrombosis associated with B12 deficiency and elevated homocysteine, which improved after stopping metformin and starting B12 [2].

The good news is that the deficiency is treatable. A 2025 randomized study found that oral methylcobalamin 1000 mcg daily was as effective as intramuscular injections for restoring B12 levels in metformin-treated patients [7].

My call: metformin clearly causes B12 deficiency in a dose- and duration-dependent manner, at rates higher than the label states, and this deficiency can cause real harm including neuropathy and elevated homocysteine. Anyone on metformin should have their B12 checked regularly and should supplement if deficient. Confidence: high.
