Is TAF (Descovy) enough for PEP gut tissue after exposure?
The short version: nothing in front of me measured TAF drug levels in gut tissue after a single PEP dose. The one study here is about adherence testing, not protection.
The record that surfaced is a dosing study in healthy volunteers, not a PEP trial [1]. It shows TAF/FTC taken daily builds up drug in dried blood spots in a dose-dependent way, and that levels level off around week 10 [1]. That is a lab marker of how much drug someone has been taking. It says nothing about how much active drug reaches the gut wall, and nothing about whether that amount blocks HIV at the moment of exposure.
Here is the gap that matters for your question. PEP needs drug present in gut tissue within hours of exposure, not at steady state ten weeks later . The blood-spot measure in this study is a proxy for adherence, not a measure of tissue penetration [1]. Nothing here compares TAF to TDF in gut tissue, and nothing here tests PEP at all.
| What the study measured | What your question needs |
|---|---|
| Drug in dried blood spots [1] | Drug in gut tissue |
| Daily dosing to steady state | Single urgent dose after exposure |
| Healthy volunteers [1] | People after a real exposure |
This is a thin retrieval for a tissue-penetration question. The animal work on TDF concentrating in gut tissue that I mentioned earlier is not in this block, so I am not re-citing it. What would settle it: a study measuring intracellular tenofovir in rectal or gut biopsy tissue after TAF versus TDF dosing.
My call: I cannot tell you TAF is enough for gut tissue from this evidence, and neither can anyone citing this study. Confidence: low.

Intracellular Tenofovir-Diphosphate and Emtricitabine-Triphosphate in Dried Blood Spots Following Tenofovir Alafenamide: The TAF-DBS Study
In a randomized crossover study of healthy volunteers, DBS TFV-DP and FTC-TP concentrations reflected TAF/FTC dosing frequency, distinguished 33%, 67%, and 100% adherence, approached steady state by week 10, and showed dose proportionality with slope greater than 1.
