What taper rate is safest to reduce withdrawal insomnia?
The short version: nothing in the evidence fixes a taper rate for withdrawal insomnia. The one concrete rate on record is a case, not a trial.
Here is what the records actually contain on schedule. A 24-year-old on 4200 mg/day stopped abruptly, got withdrawal insomnia on day one, and had a seizure on day three [2]. Symptoms eased once pregabalin was restarted at 300 mg/day and cut by 25 mg per day [2]. That is a single patient, and the reduction was 25 mg daily regardless of the starting dose's size. It tells you a gradual stepwise taper settled him, not why.
The rest of the retrieval is about pregabalin's effect on sleep, not its withdrawal. Pregabalin during benzodiazepine withdrawal improved sleep scores, and the effect was partly independent of anxiety reduction [4]. In fibromyalgia trials it improved sleep disturbance [3]. Neither bears on the taper rate question.
No record compares a fast taper against a slow one. No record measures insomnia as an outcome across different reduction speeds. So any specific percentage-of-dose-per-week recommendation would be invented. What exists is a general warning that abrupt stops are dangerous and tapering is preferred [1].
| What the records give | Detail |
|---|---|
| Abrupt stop outcome | Insomnia day 1, seizure day 3 [2] |
| Taper that worked | 300 mg restart, cut 25 mg/day [2] |
| Evidence for taper speed | None comparative |
My call: the taper should be slow enough that withdrawal symptoms, including insomnia, settle between steps, and this evidence cannot name a number. Confidence: not clear, because no study here compares taper rates on insomnia as an outcome.

Psychiatric Symptoms Following Sudden Withdrawal from the Drug Pregabalin
Doctors and pharmacy staff should be aware that psychiatric symptoms may occur after sudden withdrawal from pregabalin, even in standard-dose patients; doses should be tapered rather than stopped suddenly.

SEİZURE TRİGGERED BY SUDDEN DİSCONTİNUATİON OF PREGABALİN: A CASE REPORT
A 24-year-old male with pregabalin dependence and no history of epilepsy developed a generalized tonic-clonic seizure three days after abruptly discontinuing chronic high-dose (4200 mg/day) pregabalin, with all investigations (labs, MRI, EEG) normal, and no seizure recurrence af…

A randomized, double-blind, multicenter, placebo-controlled phase III trial to evaluate the efficacy and safety of pregabalin in Japanese patients with fibromyalgia
This randomized, double-blind, multicenter phase III trial in Japan shows that pregabalin up to 450 mg/day significantly reduces pain and improves sleep and functioning in Japanese patients with fibromyalgia, with a manageable safety profile.

Effects of Pregabalin on Subjective Sleep Disturbance Symptoms during Withdrawal from Long-Term Benzodiazepine Use
Pregabalin treatment during benzodiazepine withdrawal was associated with clinically relevant improvements across all MOS Sleep Scale dimensions and the summary index, and these sleep improvements appeared partly independent of anxiety reduction.
