Tee tree oil
Tea tree oil works for some skin conditions but the evidence is thin, the formulations vary wildly, and the allergic risk is real and dose-dependent.
The evidence for tea tree oil comes mostly from small, short-term studies, often without a proper placebo arm. For acne, a 5% gel reduced lesions by about 44-55% in two small trials, but the adverse event rate was 10-44% [1]. A 2016 open-label pilot study of 18 people showed a 55% reduction in lesions over 12 weeks, but with no control group it tells you nothing about whether the improvement came from the oil or just time [5]. The antifungal data is weaker: a 1992 trial of 10% cream for athlete's foot found a 30% mycological cure rate, barely better than placebo's 21% and far below the 85% rate of the drug tolnaftate [12]. For MRSA decolonization, a 2004 trial of 236 patients found tea tree oil cleared 41% of patients versus 49% for the standard regimen, and it was significantly worse at clearing nasal carriage (47% vs 78%) [13].
The allergic contact dermatitis risk is real. A 2003 predictive patch test in 311 healthy volunteers found about 1% developed a potential allergic reaction to pure tea tree oil [9]. A 2016 case report documented a woman who developed a severe allergic rash from a tea tree oil hydrogel used on a burn [8]. The main sensitizer appears to be ascaridole, an endoperoxide that activates dendritic cells and triggers inflammatory cytokines [10]. Oxidation over time increases the allergenic potential, so old or improperly stored oil is riskier.

The 2025 computational study flagged that tea tree metabolites may have endocrine-disrupting potential, binding to androgen and thyroid receptors in silico [16]. This is a theoretical risk from computer modeling, not clinical evidence, but it is a signal that deserves real-world testing, which has not been done.
My call: tea tree oil has modest, real but inconsistent efficacy for acne and some fungal conditions, with a non-trivial allergic risk that increases with oxidation and concentration. The evidence base is too small and too poorly designed to recommend it over standard treatments, and the long-term safety data, especially for endocrine effects, simply does not exist. Confidence: low.
