The cholesterol story is much less settled than you have been told. The diet-heart hypothesis is a theory, not a proven fact, and the evidence for red meat and eggs is mixed enough that the standard warnings do not hold up for healthy people.
The core claim—that high LDL cholesterol causes heart disease and early death—fails a key test. In large studies of elderly people, the opposite is consistently observed: higher LDL is linked to lower all-cause mortality, not higher [3]. A 2020 study of 4,485 dyslipidemic patients found that very low LDL levels were associated with a nearly doubled risk of dying from any cause (OR 1.96, CI: 1.22-3.16), yet no increase in cardiovascular events [1]. A 2015 Chinese study of 935 people aged 80 and older found that each unit increase in LDL was tied to a 19% reduction in three-year mortality (HR 0.81, CI: 0.71-0.92) [2]. These are not outliers; they are part of a pattern the cholesterol hypothesis does not predict.
For red meat, the evidence is observational and small in effect. The strongest link is with colorectal cancer, with a lag of roughly 15-20 years between consumption and rising cancer rates [9]. But a systematic review of ischemic heart disease found no clear relationship: some studies show a protective effect, others a small increase, and most fail to reach statistical significance [10]. Processed meat has a stronger, more consistent cancer signal (about 19% increased risk per 50 grams daily for pancreatic cancer) than fresh red meat [8]. The recent meta-analysis of randomized controlled trials found that substituting red meat with high-quality plant protein improves blood lipids, but no trial has shown that reducing red meat alone extends life [11].
Eggs are perhaps the clearest case of an overblown risk. A 2017 meta-analysis found that eating eggs raises both LDL and HDL by similar amounts (about 5-6 mg/dL each), leaving the critical ratio unchanged [7]. The SUN Project followed 14,185 people for 6 years and found no association between egg consumption and cardiovascular disease (HR 1.10, CI: 0.46-2.63 for highest versus lowest intake) [4]. The massive China Kadoorie Biobank study of 0.5 million people found that moderate egg consumption was associated with a lower risk of cardiovascular disease and stroke [6]. The strongest contrary signal comes from a U.S. study of older adults, where egg consumption was linked to higher CVD risk, but only in people with diabetes [5].
