Tell me about the shingles vaccine and benefits for dementia
The evidence for a dementia benefit from the shingles vaccine is suggestive but built on observational studies with serious design problems.
A 2025 systematic review and meta-analysis found that people who received any shingles vaccine had a lower risk of developing dementia compared to unvaccinated people (hazard ratio about 0.68) [1]. That sounds large, but the analysis had extreme statistical heterogeneity (I² = 99%), meaning the individual studies disagreed wildly with each other [1]. When the researchers removed studies that did not adjust for key confounders, the effect shrank [1]. This is a classic sign that the apparent benefit may come from "healthy vaccinee bias" — people who choose vaccination also exercise, eat better, and have better access to healthcare, all of which lower dementia risk independently.
A separate 2025 Delphi consensus ranked the shingles vaccine as a top candidate for repurposing to prevent Alzheimer's disease [2]. That is expert opinion, not proof. The panel recommended running a proper randomized trial through the PROTECT platform, which has over 40,000 participants [2]. No such trial results are in the evidence here.
The mechanism is plausible: the varicella-zoster virus can reactivate and cause inflammation in blood vessels and brain tissue, and vaccination may prevent that trigger [1]. But plausible mechanism is not evidence of effect.
My call: the shingles vaccine may reduce dementia risk, but the evidence is too weak and conflicted to rely on. A randomized trial is needed and has not been done. Confidence: low.
