Tetanus vax is harmfull?

Nov 2, 2026 · 7 sources used · OpenNeedle synthesis
The short version: the tetanus shot is one of the better-studied vaccines, and the harms it reliably causes are mostly local and short-lived. The harms that would matter most are the ones nobody here measured.

The common harms are real but small. In a 2025 study of 485 pregnant women, about 44 in 100 who got Tdap had injection-site pain, versus fewer with plain tetanus toxoid, and severe pain was rare [1]. In adolescents, pain ran about 75 to 78 in 100, severe pain about 5 to 18 in 100, and fever about 5 in 100 [5]. A second Tdap given 4 to 5 years later brought pain in about 88 in 100 and myalgia in about 61 in 100 [6]. One serious reaction, Arthus (a severe local inflammatory reaction at the injection site), was estimated at about 0.1 per million doses [2].

Adolescents, Adacel [5]78%18%5.0%

The safety record rests heavily on passive reporting. The DTaP review of 50,157 VAERS reports found no new events, but VAERS is voluntary and catches a fraction of what happens [4]. The VSD finding of no link to neurologic events is stronger [3], but it is surveillance, not a trial. No record here compares vaccinated people to unvaccinated people over years for hard outcomes. That comparison is the one that would settle long-term harm, and it is not in front of me.

One signal worth naming: in very preterm infants vaccinated in the NICU, 20 in 100 had major cardiorespiratory events, and vaccination at 70 days or younger tracked with those events [7]. That is a specific, fragile population, not your healthy adult.

My call: for a healthy adult or child, the tetanus shot's documented harms are local and transient, and the disease it prevents kills. For a very premature infant, timing deserves a real conversation with the clinician. Confidence: moderate on short-term harms, low on long-term, because the long-term vaccinated-versus-unvaccinated comparison is not in this evidence.

A large Vietnamese prospective cohort study assesses the safety of maternal Tdap vaccination during pregnancy, finding safety similar to tetanus toxoid with more local injection-site pain but no major safety concerns, and demonstrating that co-administration with inactivated inf…
