Are there alternative chemo or targeted options that may be less toxic?
The short version: the evidence retrieved does not contain a single trial that directly compares a less-toxic alternative to FOLFOX in metastatic duodenal adenocarcinoma. Your question is the right one; the literature has not answered it.
The evidence you need would be a randomized trial in duodenal cancer comparing FOLFOX to, say, a fluoropyrimidine alone, a taxane-based doublet, or a targeted regimen matched to a genomic alteration. That trial does not appear in the records retrieved. What exists instead is a 2019 Korean phase II trial in metastatic gastric cancer that found paclitaxel/5-FU produced only 34% grade 3/4 adverse events versus 72% for FOLFOX, with no difference in overall survival [12]. That is a nearby population, not yours, but it suggests a taxane doublet may be less toxic and equally effective. A 2021 study of 68 pancreaticoduodenal cancer patients who had failed standard therapy found that next-generation sequencing identified actionable mutations in 30 patients, and 10 received matched targeted therapy, with two case reports showing partial responses to sorafenib in RAF-pathway altered duodenal adenocarcinoma [3]. That is a salvage setting, not first-line, but it shows that genomic testing can open a less-toxic door.
