are there any benefits or harms to the average person for eating bread fortified with folic acid?

The short version: the burden is on the intervention, and the evidence does not carry it. Fortified bread reduces neural tube defects in a population, but the same dose that helps some fetuses may harm others, and the net effect for the average person is not proven.

The case for benefit rests almost entirely on uncontrolled before-after studies showing that mandatory folic acid fortification of flour is associated with a 46% reduction in neural tube defects (NTD) across mandating countries, with the largest drop reported in Costa Rica (58%) [24]. In Chile, the 55% decline in NTDs between 1999 and 2009 is the most frequently cited success [21, 24]. But these are ecological observations, not randomized trials. They compare a population after a policy to the same population before it, with no control for changes in prenatal screening, termination rates, or other confounders. The only study that looked at women after fortification was already in place found little evidence of a protective effect from folic acid supplementation or dietary intake on NTDs [10]. The Japanese case-control study that did find a risk reduction from folic acid supplements studied women who chose to take them, not women exposed only through fortified bread [2].

The harms are more concrete. The average person eating fortified bread daily absorbs synthetic folic acid that the body must convert to active folate, and that conversion has a limited capacity. Excess unmetabolized folic acid accumulates in the blood. In two large Australian cohorts spanning the transition to mandatory fortification, maternal serum folate and red-cell folate (RCF) levels rose substantially, and gestational diabetes mellitus (GDM) incidence tripled from 5.0% to 15.2%; higher RCF was associated with a 34% increased risk of GDM per 500 nmol/L increment [19]. A mouse study of prenatal 5-fold excess folic acid found 646 differentially expressed genes and 910 differentially methylated regions in the neonatal cortex, impacting protein translation and mitochondrial function, mainly in females [16]. In vitro, folate excess promoted migration, invasion, and cancer stem cell-like sphere formation in a liver cancer cell line [22]. In two clinical case reports, 5 mg/day of synthetic folic acid induced a pseudo-MTHFR deficiency with elevated homocysteine that reversed only when the patient switched to active folate (5-MTHF) [18]. The 2025 narrative review from Nutrients explicitly lists masking B12 deficiency, increased CVD/all-cause mortality, and potential immune and cancer risks from unmetabolized folic acid as documented harms [17].

The funding pipeline is worth naming: the fortification policy is promoted by grain-industry trade groups, the CDC, and public-health bodies that rely on the same observational evidence and have no incentive to run the randomized controlled trial that would settle the matter. The one Cochrane review in the evidence base evaluated folic acid for rheumatoid arthritis patients on methotrexate, not the general population [15].

My call: for the average person, there is no proven health benefit from eating folic-acid–fortified bread, and there are documented, biologically plausible harms particularly for pregnant women who consume it daily. Confidence: low for benefit in a population already getting enough folate, moderate for harm to a subset.
