Is there any evidence for the efficacy of DMSO in managing endometriosis?
The short version: there is no direct evidence that DMSO treats endometriosis in humans. The retrieved studies test it for bladder pain, arthritis, and infections, not for endometrial lesions or pelvic pain from endometriosis.
The evidence you retrieved is almost entirely off-topic. One 2016 trial tested intravesical DMSO against hyaluronic acid for bladder pain syndrome, not endometriosis [1]. DMSO reduced pain but was not compared to placebo, and the trial was open-label, meaning both patients and doctors knew who got what. That design inflates perceived benefit. A 2017 review of DMSO for interstitial cystitis reports response rates of 86-95% in small uncontrolled studies from the 1970s and 1980s [2]. Those are not modern evidence. A 2002 rat study shows DMSO suppresses spinal nerve signals from bladder distension [4], which is a plausible mechanism for pelvic pain relief, but it is an animal model, not a human trial.
The 1983 trial in rheumatoid arthritis [5] and the 2011 pooled safety data for topical diclofenac with DMSO [6] are for different conditions. The mouse toxicology studies [7, 8] show DMSO can cause apoptosis in lymphoid tissue and death at high doses, but those doses are far above any topical or intravesical human use. None of this answers whether DMSO reduces endometriosis lesion size or pain in a woman with the disease.
My call: the evidence does not support using DMSO for endometriosis. Confidence: high that no human trial exists. The mechanism is plausible but untested for this condition. If you are considering it, you are taking an unstudied risk for an unproven benefit.
