The short version: the evidence for a vaccine-autism link is weak, but the evidence on autoimmunity is real and specific, and the long-term safety studies you would want simply have not been done.

The autism question has been studied more than any other vaccine safety question. The large studies here, including a meta-analysis of over a million children, find no association between vaccines and autism [1]. A Japanese study is the most telling: when MMR was withdrawn entirely, autism rates kept rising [2]. That is a natural experiment, and it points away from vaccines as a cause. The 2024 VAERS analysis that flagged autism reports after MMR and hepatitis B vaccines is a passive reporting system, which detects almost nothing by design and cannot establish causation [3]. The 2021 study showing vaccinated children had higher rates of autism, ADHD, and asthma compared unvaccinated children, but it was a small, self-selected sample with odds ratios so extreme they strain credibility [4]. Take it as a signal worth investigating, not as proof.

ADHD specifically has almost no dedicated research. The 2021 study is the only one here that even mentions it, and its design is too weak to trust [4]. The evidence is simply not there.

Autoimmunity is different. The French cohort of over 2 million girls found HPV vaccination was associated with a nearly fourfold higher rate of Guillain-Barré syndrome, a serious autoimmune nerve condition [6]. Case reports document lupus after HPV vaccine [8] and bullous pemphigoid after COVID vaccines [9]. A 2025 study found post-vaccination syndrome causes cognitive impairment and fatigue similar to long COVID, with elevated inflammation markers [5]. The mechanism is plausible: adjuvants and spike proteins can disrupt immune balance and trigger autoimmune cascades. The Danish study on type 1 diabetes found no link [7], but that is one disease.

The studies on autism are mostly funded by or conducted within the same institutions that promote vaccination. The autoimmune case reports are independent. The most important gap: no long-term safety study has ever compared fully vaccinated to completely unvaccinated children over decades. That study would settle this. It has not been done.

My call: for autism, the evidence does not support a link, and I say that as a skeptic. For ADHD, the evidence is absent. For autoimmunity, there are genuine, specific signals that deserve far more investigation than they have received. Confidence: moderate.
