Is there a cure for tinnitus
The short version: there is no cure for tinnitus. The evidence shows ways to manage the distress, not to make the sound stop.
The evidence you have is about managing the suffering, not the sound itself. A 2024 systematic review of 15 randomized trials found that tinnitus retraining therapy is not superior to other interventions [7]. Cognitive behavioral therapy (CBT) can reduce how much the noise bothers you. One study showed CBT dropped the Tinnitus Handicap Inventory score from 58 to 22, a meaningful improvement in quality of life [6]. But the sound was still there. A meta-analysis of wait-list control groups found that even doing nothing leads to a small improvement over time in some people [3]. That tells you how much of the distress is driven by anxiety and attention, not by the ear.
For a small subset with a physical cause, there is a fix. Middle ear myoclonus, a muscle spasm in the ear, resolved completely with medication in 25% of patients in one case series and with surgery in nearly all of them [1]. But that is a rare mechanical problem, not the common tinnitus that millions live with.
The most honest answer is that the medical system has no drug or procedure that reliably turns off the phantom sound. The interventions that work, CBT and sound therapy, teach you to live with it. That is not a cure. It is accommodation.
My call: there is no cure for tinnitus. The best evidence supports cognitive behavioral therapy to reduce the distress, not to eliminate the sound. Confidence: high.
