Is there evidence that changes in air pressure can contribute to chronic tension or any other chronic headache?
The short version: the evidence shows falling or low barometric pressure triggers measurable physiology changes, and the same pattern appears across several pain conditions, but directly linking air pressure to chronic headache as a distinct disorder is inferential, not proven.
Of the 32 studies retrieved, only one directly tested weather-pressure effects on headache in a controlled setting. The 2021 crossover pilot study [16] put 16 healthy adults in a pressure chamber and found that a 20 or 40 hPa drop produced ear pressure, head compression, and mild headache in three of 15 subjects. That is a short, transient effect in healthy people, not proof of chronic headache amplification in an already-pain-prone population. It does show the mechanism exists: the baroreceptors in the ear and sinuses are sensitive enough to report pressure changes as discomfort.
The circumstantial case is broader. A dental emergency study [13] found a strong correlation (r=0.75) between day-to-day barometric pressure deviations and visits for acute oral pain, especially tooth abscess and periodontitis. The periodontal study [11] found that a maximum hourly pressure drop above 1.5 hPa predicted a 1.3% incidence of acute periodontal symptoms two days later, versus a lower baseline rate. Menière's disease patients [23] had a 30% higher odds of an attack when pressure was below 1013 hPa. Stroke studies consistently find that falling pressure or low absolute pressure predicts vascular events, including ischemic stroke [2] and hemorrhagic stroke [29].
The mechanical pathway is straightforward but never tested for headache specifically: pressure drops cause gas trapped in closed body cavities (sinuses, middle ear, intracranial air bubbles) to expand [6]. That expansion stretches pain-sensitive tissue. The same physics drives barometric pumping in landfills [17] and nuclear test sites [12]. There is no reason the sinuses and ear canals would be exempt.

The remaining 25 records are irrelevant to your question: they study speech aerodynamics, plant microbes, landfill gas, insects, beef embryos, plasma jets, wheat yields, and fire risk. That is a massive signal-to-noise problem in the retrieved evidence. The studies that do matter share a real limit: they are all short-term correlations or acute provocation, never a prospective design tracking chronic headache patients through weeks of weather variation.
My call: the evidence supports air pressure change as a plausible trigger for acute head sensations and for exacerbations of several pain conditions, but there is no dedicated study that proves it drives chronic tension headache as an ongoing disorder. The mechanism exists, the circumstantial data is consistent, and the controlled human data is thin but positive. High confidence on the plausibility and acute trigger effect; low confidence that it explains chronicity, because nobody has run that study.
