How useful is exercise for a 68yr old morbidly obese woman
The short version: exercise is one of the few interventions that reliably helps a 68-year-old morbidly obese woman, but the dose and type matter more than the label.
The evidence here is unusually consistent. An umbrella review of frail older adults found multi-component exercise, especially with resistance training, improves strength, gait speed, balance, and overall physical performance [2]. A meta-analysis of 72 trials in overweight and obese people found exercise improves cardiorespiratory fitness, blood pressure, and insulin resistance, with the best results from programs of 12 weeks or less, three or more sessions weekly, and sessions under 60 minutes [29]. For an older obese woman, the risk is not the exercise itself. The risk is doing nothing: a large Finnish cohort found people who rated their fitness poor had roughly double the death rate from heart disease, stroke, respiratory illness, and dementia compared to those who rated it good [31].
The mechanism matters. In obese older adults, 12 weeks of supervised aerobic exercise raised a hormone called GDF15, which tracked with reduced fat mass, lower triglycerides, and better insulin sensitivity [35]. Combined aerobic and resistance training during weight loss preserved muscle protein synthesis and lean mass better than either alone [41]. That is the real target: losing fat while keeping muscle. Diet alone in older adults can strip lean mass, which is why the combination of exercise and calorie restriction beats either alone [32].
The funding picture is cleaner than most medical claims because exercise has no patent holder. No manufacturer is paying for these trials. The main limitation is adherence: only about 17 in 100 people meet physical activity guidelines, and the benefits vanish if the program stops [32]. For a 68-year-old morbidly obese woman, the practical answer is supervised, multi-component exercise, started gently, with resistance work prioritized, and paired with adequate protein. The evidence does not support high-intensity interval training as a first choice here; the trials showing HIIT benefits were mostly in younger or less obese populations [21, 28].

Confidence: high for functional and metabolic benefits, moderate for long-term weight loss, which depends on adherence and diet.
