Is viagra safe
The short version: sildenafil is a vasodilator with well-documented side effects, and the evidence here is almost entirely from industry-funded or short-term studies that do not answer the long-term safety question for a healthy person taking it recreationally.
The evidence you have is a stack of studies funded by or closely tied to the manufacturers (Pfizer, Lilly, Bayer). The 1998 safety paper [9] and the 2004 post-marketing surveillance [5] are the closest to a broad safety picture, and both report the same pattern: headache in about 16-25% of users, flushing in 10-30%, nasal congestion, dyspepsia, and visual changes [9, 11]. A 2000 clinical-practice study found higher rates than the industry trials: 30.8% flushing, 25.4% headache, 18.7% nasal congestion [11]. The cardiovascular meta-analyses [10, 13] claim no excess of heart attacks or death, but they compare sildenafil users to placebo users in short trials, not to unmedicated men over years. The myocardial infarction rate was 1.7 per 100 man-years on sildenafil versus 1.4 on placebo [9] — a small absolute difference, but the trials excluded men on nitrates and those with unstable heart disease, so the real-world risk in someone who does not know they have coronary disease is unknown.
The most important safety rule is absolute: never combine sildenafil with nitrates (nitroglycerin, isosorbide). The interaction causes severe, potentially fatal hypotension [7, 10]. That is not a theoretical risk; it is a labeled contraindication backed by the pharmacodynamics. A man who takes sildenafil and then uses a nitrate for chest pain is in danger. The evidence also shows that a high-fat meal delays absorption and reduces peak blood levels by about 29% [15], which means the drug hits harder on an empty stomach — relevant if someone takes it impulsively.
What is missing from this evidence set is any long-term safety study in healthy men taking it intermittently for years. The longest trials here run 6-12 months [6, 28]. The rare but serious adverse events reported in the literature — nonarteritic anterior ischemic optic neuropathy (vision loss), sudden hearing loss, priapism — are not captured in these short trials because they are too rare to show up in a few hundred or few thousand patients. The 1998 safety paper [9] reports "abnormal vision" in 3% of users, which is the PDE6 cross-reactivity in the retina, but that is not the same as the ischemic optic neuropathy case reports.

My call: for a man with diagnosed erectile dysfunction who has been cleared for sexual activity by a cardiologist and is not on nitrates, sildenafil is reasonably safe in the short term, with predictable and mostly tolerable side effects. For a healthy person taking it recreationally or without a cardiac evaluation, the risk-benefit is worse — the benefit is zero and the risks (headache, flushing, vision disturbance, and the unknown long-term cardiovascular signal) are real. Confidence: moderate for short-term safety in screened patients, low for long-term safety or recreational use.
