Whats the deal with parasites? Does a person need to address them medically?

The short version: parasites are common, often harmless, and the evidence for routine medical treatment is thin outside of clear disease or high-risk groups.

The evidence here is almost entirely about livestock and wildlife. You have studies on deworming heifers, horses, reindeer, goats, and dogs [1, 3, 4, 6, 7, 24]. One study on oregano oil in 14 symptomatic patients showed some improvement [2]. Another found that HIV patients on antiretroviral therapy had far fewer parasites, suggesting a healthy immune system usually handles them [8]. The human studies that exist are cross-sectional surveys, not treatment trials. They report prevalence, not whether treating makes people better [10, 25, 27, 28, 29, 31, 32, 33, 34, 35, 36].

The most interesting finding is that some parasites may be protective. A 2020 study in Indian adults with type 2 diabetes found that those infected with Strongyloides stercoralis had a dampened inflammatory profile, and deworming partially reversed that [11]. Another study showed that a lower dose of Toxoplasma infection produced better long-term immunity with less exhaustion [16]. The filarial infection study found that infected people had monocytes in an anti-inflammatory, regulatory state [14]. This is the hygiene hypothesis in action: the immune system evolved with parasites, and removing them can sometimes backfire.

For a generally healthy person in a developed country, the risk from a common intestinal parasite like Blastocystis or Endolimax nana is close to zero. The risk from a drug like albendazole or ivermectin is not zero: it can cause liver toxicity, neurological effects, and gut disruption. The burden of proof is on the person who wants to give you the drug, not on you to prove you are fine without it.

My call: for a healthy person without symptoms, the evidence does not support routine screening or treatment. Confidence: moderate.
