Which Covid vaccine is most effective given current variants show specific vaccines and rank effectiveness show novovax against mRNA
The short version: the evidence here does not tell you which COVID vaccine is most effective against current variants, because every study in this batch tested older variants and none directly compared Novavax to an mRNA vaccine.
The best comparison in the evidence is between mRNA vaccines and viral-vector vaccines. A Slovenia surveillance study from October 2021 reported that mRNA vaccines gave 92–93% protection against COVID hospitalization in adults under 65, while vector vaccines gave 76–82% in the same age groups [16]. Protection dropped sharply in people over 65, especially six months after vaccination, to 43% for vector recipients [16]. A Turkive study of 24,538 patients concluded that two or more mRNA doses reduced ICU admission by 99% compared to a single inactivated dose in older adults [7].
But Novavax is almost absent. Only one phase 1-2 trial appears here, showing it generated strong antibody and T-cell responses in 2020 against the original strain [1]. No study in the evidence measures Novavax against Omicron subvariants, against mRNA vaccines head-to-head, or on hard outcomes like hospitalization or death in the current viral landscape. A high antibody titer is a surrogate endpoint, not proof of real-world protection.

The safety signal in the evidence is not reassuring. Multiple case reports link mRNA vaccines to severe hematologic and cardiac events: thrombotic thrombocytopenic purpura [5], aplastic anemia in a 15-year-old and a 60-year-old [6, 9], and fulminant myocarditis in a 19-year-old [8]. These are case reports, not population rates, but they establish plausible severe injury pathways that no Novavax comparison can rule in or out.
My call: the data for current variants is too thin to rank Novavax against mRNA vaccines. The older evidence favors mRNA over vector vaccines for severe-disease prevention, especially in younger adults, with clear waning in the elderly. The absence of any head-to-head trial of Novavax versus mRNA on current variants means the most informative comparison was never run. Confidence: low that any ranking from this evidence applies to today's variants.
