Which inflammatory markers change most with diet, and are they consistent?
The honest answer: the most diet-responsive inflammatory markers are C-reactive protein and fasting insulin, but the consistency is modest and the clinical translation is unproven.
The best retrieved evidence is a 2013 systematic review and meta-analysis of randomized trials comparing low glycemic index/load diets to high glycemic index/load diets [1]. Across the included trials, the low-GI pattern lowered C-reactive protein by a weighted mean difference of 0.43 mg/L (95% CI −0.78 to −0.09, p=0.01) and fasting insulin by 5.16 units (95% CI −8.45 to −1.88, p=0.002) [1]. Those are the two markers with statistically significant, directionally consistent changes.
But the consistency has limits. The same analysis found a small but significant loss of fat-free mass (−1.04, 95% CI −1.73 to −0.35, p=0.003) and only a borderline weight change (−0.62, 95% CI −1.28 to 0.03, p=0.06) [1]. So the diet lowered two inflammatory markers while also costing lean tissue, which is not a clean win. And the review pooled trials with heterogeneous designs, durations, and patient populations; the confidence intervals are wide, and the p-values are not overwhelming.
What the retrieval does not show: any randomized trial where a dietary change reduced hard clinical events like heart attacks or death through an inflammation pathway. The markers move, but whether that movement matters for outcomes is not established by these records.
