Will folic acid cause harm

The short version: folic acid is not harmless at high doses, and the evidence shows real risks that are rarely mentioned.

The strongest documented harm is masking B12 deficiency. After the US added folic acid to flour, the share of older people with low B12 but no blood abnormality rose from 70% to 87% [4]. That means B12 deficiency goes undetected until nerve damage is done. The 2022 systematic review found that nearly all women taking folic acid supplements in fortified countries exceed the 1000 mcg upper limit [5]. High-dose folic acid (5 mg/day) also raised homocysteine in a wild-type patient, a pseudo-MTHFR syndrome reversible only by switching to methylfolate [13].

The immune signal is concerning. Postmenopausal women with unmetabolized folic acid in their blood had lower natural killer cell cytotoxicity, a 23% relative drop [9]. A meta-analysis of 10 trials found a borderline significant increase in overall cancer with folic acid supplementation [7]. The 2023 analysis found folic acid combined with aspirin led to more cognitive decline, not less [17].

The evidence base is thin on long-term outcomes. Most studies use blood markers, not disease or death. The 5-MTHF form avoids unmetabolized folic acid entirely and shows higher bioavailability with lower UMFA exposure [2]. If you supplement, methylfolate is the safer choice, and check B12 first.

My call: folic acid at high doses or in fortified countries carries real, documented risks, especially for B12 masking and immune function. Confidence: moderate.
