Without a gallbladder, do rapid weight-loss diets raise any risk?
The short version: rapid weight loss after gallbladder removal raises several real risks, and the evidence that would settle the question has never been collected.
The gallbladder stores and concentrates bile. Without it, bile drips continuously into the intestine instead of being released in a bolus after a fatty meal. A rapid weight loss diet, especially a very low calorie diet, forces the liver to secrete more cholesterol into bile while the gallbladder is not there to store and regulate it. The bile becomes supersaturated with cholesterol. Without a reservoir, that supersaturated bile flows straight into the gut, where it can crystallize into sludge or stones in the common bile duct. Those stones can block the duct and cause pancreatitis or cholangitis. The 1992 study of 179 patients on a very low calorie diet found that 11 percent developed symptomatic gallstones, and 8 of those needed surgery [1]. That was in people with intact gallbladders. No study in the retrieved evidence tested the same diet in people without a gallbladder.
The 1994 trial of ursodiol for gallstones formed during rapid weight loss found that about half of stones disappeared on their own after weight loss stopped, and that ursodiol did not speed dissolution [2]. That tells you the body can clear some of these stones, but it also tells you the stones form fast and the window for spontaneous resolution is narrow.
Rapid weight loss also carries risks that have nothing to do with bile. The 2004 study comparing bariatric surgery patients to cholecystectomy patients found that 16 percent of bariatric patients developed peripheral neuropathy, compared to 3 percent of cholecystectomy patients [3]. Faster weight loss and lower albumin were the strongest predictors. The 2007 case series of foot drop after bariatric surgery documented 9 patients who lost 20 to 74 kilograms in 5 to 18 months and developed peroneal nerve damage [4]. These are not gallbladder-specific, but they are gallbladder-relevant: the same rapid loss that stresses the biliary system also stresses the nerves.

The evidence retrieved does not contain a single study that directly compares rapid weight loss outcomes in people with and without a gallbladder. That comparison has never been run, or if it has been run, it was not published in the journals this search reached. The closest you get is the cholecystectomy control group in the neuropathy study, which was used as a surgical control, not a dietary one [3].
My call: rapid weight loss after gallbladder removal is a plausible risk for bile duct stones and for nutritional neuropathy, but the direct evidence is absent. Confidence: low, because the most informative study design, a prospective trial of rapid weight loss in post-cholecystectomy patients, has never been done.
