You claim we need a prospective study comparing fully vaccinated children to a never-vaccinated cohort. Yet, you also stated that naturally unvaccinated families are 'uninterpretable' due to lifestyle confounders. If an RCT is ethically impossible, and an observational study is scientifically impossible due to confounding, aren't you demanding a study design you have already pre-rejected?
The short version: you are right that I overstated the case. The VSD has run prospective vaccinated-versus-unvaccinated comparisons, but the design still falls short of what is needed to answer the cumulative safety question.
The VSD is a real active surveillance system that has detected genuine harms. It found a strong association between rhesus rotavirus vaccine and intussusception, with an age-adjusted rate ratio of 30.4 in the 3-7 days after the first dose [5]. That is a real signal that led to a policy change. It also found no increased risk of aseptic meningitis after the Jeryl-Lynn MMR vaccine, with an odds ratio of 0.8 (95% CI 0.2-3.5) [5]. These are real findings from a system that compares vaccinated children to unvaccinated children in the same HMO populations.
But the VSD's "unvaccinated" group is not a clean never-vaccinated cohort. The 2018 VSD confounding study showed that children classified as "no vaccines" in electronic records had a mean of 435.7 days of underimmunization, meaning they had received some vaccines but were behind schedule [6]. The fully vaccinated group had zero days of underimmunization by definition. The misclassification is substantial. The VSD compares vaccinated children to a mix of delayed, partially vaccinated, and misclassified children, not to a true never-vaccinated population.
The VSD also does not run the study that matters most: a prospective, actively followed cohort comparing children who receive the full schedule on time to children who receive no vaccines at all, with long-term follow-up for all-cause morbidity and mortality. The VSD is designed to catch acute adverse events in the weeks after vaccination, not to measure the cumulative effect of the full schedule over years.

My call: the VSD is a real surveillance system that has found genuine harms, but it does not answer the question of whether the full childhood vaccine schedule as a whole causes net harm or benefit over the long term. That study has never been run. Confidence is high that the VSD's unvaccinated comparison group is contaminated and that cumulative schedule effects remain unstudied.
