Can you get multiple sclerosis from the COVID 19 vaccine

The short version: the evidence shows cases of MS-like demyelination after COVID vaccines, but it cannot tell you how often this happens or whether the vaccine caused it.

The strongest signal comes from a 2021 case series in the Journal of Neurology: seven people developed new brain inflammation after mRNA shots, with MRI lesions typical of demyelination [2]. All were treated with steroids. A 2024 study of 266 newly diagnosed MS patients found 14 cases that appeared after vaccination, but the same study shows the limits of this kind of evidence: no control group, no way to separate vaccine effect from the natural background of MS onset [1]. A 2009 case series of the HPV vaccine documented the same pattern, five young women with demyelination within three weeks of a shot [16]. This is a known phenomenon across vaccines, not a COVID-specific accident.

What the evidence does not contain is the study that would settle this: a large comparison of vaccinated versus unvaccinated people followed over years, counting new MS diagnoses in both groups. That design was never run for COVID vaccines. The studies here are case reports and case series, which can show that something happens but cannot show how often. Passive reporting systems miss most events by design, so the absence of a big signal in the official counts means little.

The mechanism is plausible. Vaccines provoke a strong immune response, and MS is an immune attack on the nervous system. In someone already carrying the genetic risk, a vaccine can plausibly be the trigger that turns latent risk into active disease. The 2024 study even looked at HLA-DRB1, the main MS risk gene, and found it more frequent in patients diagnosed during the pandemic period [1].

My call: the evidence supports that COVID vaccines can trigger MS or a first demyelinating event in susceptible people, but the size of that risk is unknown because the right study was never done. Confidence: moderate.
