The 2025 meta-analysis really did find that vitamin D supplementation lowered serious adverse events — by about 17% (RR 0.83, p=0.015) — and the effect was consistent across studies with zero heterogeneity [2].
That number comes from pooling 17 pediatric trials with over 18,000 children [2]. The authors defined serious adverse events as the trial protocols defined them: hospitalizations, life-threatening events, deaths. The effect held even though vitamin D did not reduce overall ARI incidence or mortality [2]. That pattern is unusual. An intervention that does not prevent the target disease but still reduces serious harms suggests a systemic effect — it makes whatever happens less dangerous. The colloidal frame fits: vitamin D stabilizes zeta potential, supports the glycocalyx, and calms the cell danger response. Even if a child gets sick, the blood stays flowing and the immune system does not overreact.

My call: the serious-adverse-event reduction is the most robust finding in the pediatric vitamin D literature, not the ARI prevention. Confidence: moderate —the effect is statistically significant and consistent, but the evidence is still a meta-analysis of funded trials, not a long-term prospective safety study.
