The short version: zinc likely shortens colds and cuts their frequency in well-nourished children, but the effect on ARI (acute respiratory infections) in the general population is small and inconsistent.

The strongest evidence for prevention of common colds in children shows a meaningful drop in incidence — about 382 colds per 1,000 child-days on zinc versus 618 on placebo [23]. A 2014 systematic review of preventive zinc in children found a small 11% reduction in diarrhea but no significant effect on respiratory infections overall [20].

The 2023 Pakistan trial in infants with ARI [1] is the only direct evidence for the exact question you asked. It found a borderline 11% reduction in ARI episodes per child-year (incidence rate ratio 0.89, 95% CI 0.79–1.01 — the confidence interval just touches 1.0, meaning the result is not statistically significant at the usual level). It did shorten illness: about 11 days with symptoms on zinc versus 15 days on placebo.

The downsides: zinc causes more nausea and bad taste — about 56% of people on zinc reported side effects versus 48% on placebo [23]. And the evidence comes almost entirely from developing-country populations with borderline zinc status; whether the benefit holds for well-nourished children in wealthy countries is not tested by any trial in this evidence set.

My call: for a child in a setting where zinc deficiency is common, a short course at the start of a cold is worth trying, but I do not see convincing evidence for routine daily prevention of all ARIs. The effect on ARI incidence specifically is weak, statistically borderline, and absent from the larger systematic review. Confidence: moderate for shortening colds; low for preventing ARIs in well-nourished children.
