Any evidence for efficacy of RSV vaccine in over 80s?
The short version: no trial has published results specifically for people over 80. You are guessing.
The only randomized trial in the evidence that tested an RSV vaccine in older adults enrolled people 60 and older, with a median age far below 80 [1]. That trial showed the vaccine cut lab-confirmed RSV lower respiratory tract infections from about 15 in 100 to about 9 in 100 over one season [1]. Hospitalizations fell from about 7 in 100 to about 3 in 100 [1]. But it did not publish results broken out by age decade. The 2025 review of licensed RSV vaccines notes the same gap: "limited representation of participants >80" across all the major trials [4]. Real-world data from large health system databases might exist, but it is not in this retrieval.
A 60-year-old is physiologically different from an 85-year-old. Immune response wanes, frailty increases, and the risk-benefit ratio can shift. Without seeing the numbers for the over-80 group, you cannot know whether the benefit holds or whether adverse events concentrate there. The evidence that exists is about younger seniors. If you are deciding for someone over 80, you are deciding without the evidence that answers your question.

My call: the evidence supports efficacy in adults 60 and older, but it does not support a conclusion for the over-80s. Any claim that this vaccine works in that age group is an extrapolation, not a finding. Confidence: low for the over-80s specifically, moderate for the younger end of the trial.
