The evidence retrieved does not answer the question of a single "base cause" of neuropathy. It instead documents a list of distinct, well-established causes.
The retrieved studies show that peripheral neuropathy has many separate triggers. Metabolic disease, specifically diabetes, is the most heavily documented cause in this set, with diabetic foot complications affecting up to 30% of patients over a lifetime and accounting for 50% of foot ulcers through pure nerve damage [16, 20]. Toxic exposures are also proven: methylmercury poisoning produced clear nerve degeneration on autopsy [2], the antibiotic linezolid caused severe sensory-motor axonal neuropathy after months of use [3], and the HIV drug stavudine tripled the risk of neuropathy compared to an alternative [6]. Chemotherapy agents like cisplatin, oxaliplatin, and vincristine all induce neuropathy through mitochondrial damage in animal models [21]. Mechanical compression from joint-related cysts caused foot drop and sensory loss in 17 surgical cases [1]. Autoimmune processes appear as well: cryoglobulinemia, often linked to hepatitis C, was found in 11% of previously undiagnosed neuropathy patients [4], and anti-ganglioside antibodies rose with neuropathy severity in diabetes [19]. Nutritional deficiency was implicated in acute neuropathy among refugees [10]. Even a high-fat diet in mice produced nerve conduction deficits and oxidative stress [17].
The evidence does not support a single root cause. It supports many independent mechanisms: metabolic toxicity, direct chemical toxicity, physical compression, autoimmune attack, and nutritional failure. The hypothesis that a single underlying factor explains all neuropathy is not supported by this retrieval. The burden of proof is on anyone claiming one cause, and the evidence here shows the opposite.
My call: the evidence shows neuropathy is a final common pathway from many distinct causes, not a single disease. Confidence: high for the claim that multiple independent causes exist; the evidence is clear on that point.
