What diet changes should I avoid in CKD stage 3 to protect kidneys?
There is one high-quality human trial on sodium restriction in early CKD, but it ran on only 20 patients, and the evidence for potassium and phosphorus change is mostly from animal studies, not from clinical trials in stage 3.
The 2013 trial of sodium restriction in CKD stage 1–2 showed that cutting sodium significantly lowered blood pressure and proteinuria [1]. A larger 2015 study found that lower sodium intake improved blood pressure control and reduced albuminuria [4]. A 2024 study of combined sodium and fluid restriction in CKD stages 3–5 showed systolic blood pressure dropping from 155 to 139 mmHg over 12 months [6]. These are real clinical benefits.
The evidence for potassium and phosphorus is thin. A 2014 fruit-and-vegetable trial in stage 3 CKD found that adding base-producing foods preserved GFR better than standard care [2], but the DASH diet pilot that also raised potassium to 4700 mg/day showed a modest increase in serum potassium and a drop in bicarbonate [3]. A 2025 mouse study found that excess dietary potassium on a normal-salt diet actually raised blood pressure through aldosterone and ENaC activation [13]. In CKD, where potassium excretion is already impaired, this matters. A high-potassium DASH diet was not designed for impaired kidneys.

My call: sodium restriction below 2 g/day is the single diet change with the strongest evidence for protecting kidneys in stage 3. Potassium and phosphorus restriction have plausible mechanisms but almost no direct human trial data in CKD stage 3. Avoid high-potassium DASH-style diets unless your labs show low potassium. Confidence: moderate for sodium, low for potassium and phosphorus.
