What is the difference between optic neuritis and NAION
The short version: optic neuritis is an inflammatory attack on the nerve; NAION is a sudden blockage of blood flow to it. They look similar but are different diseases with different treatments.
Optic neuritis is an autoimmune inflammation of the optic nerve, often linked to multiple sclerosis. It typically hits younger adults, causes pain with eye movement, and vision loss that often improves on its own. The evidence here shows most people recover good vision: about 74% reach 20/20 or better at 10 years, and 92% reach 20/40 or better [4]. Steroids speed recovery but do not change the final vision [7]. NAION is different. It is a stroke of the optic nerve head, caused by poor blood flow, not inflammation. It usually hits older people, often those with high blood pressure or diabetes, and causes sudden, painless vision loss that does not improve much. The one case report here shows it can follow a severe COVID infection, with disc swelling and altitudinal field defects [11].
The distinction matters because the treatments are opposite. Optic neuritis responds to anti-inflammatory drugs; NAION has no proven treatment. The evidence base for NAION is thin, mostly case reports [11], while optic neuritis has decades of trial data [4, 7]. The retrieval here is heavily weighted toward optic neuritis, so the NAION side rests on much weaker ground.
