The short version: the standard definition of Graves' disease is solid, but the evidence emphasises that disease progression and complications mean much more than just a lab value.

Your hypothesis is the textbook picture and it is broadly correct. Graves' disease is driven by autoantibodies against the TSH receptor, and those antibodies directly cause the overproduction of thyroid hormone [22, 26]. The 2013 study you have in front of you shows that in 52 patients, levels of inflammatory markers like IL-6, IL-12, IL-18, and blood-clotting factors like PAI-1 and vWF are all significantly elevated, confirming that the disease is not just a thyroid problem but a systemic inflammatory state that damages blood vessels [3]. The disease also has a strong genetic component, with variants in HLA, CTLA-4, and the TSH receptor itself all showing up as risk factors across populations [27].

But here is what matters more than the textbook definition. The disease can swing unpredictably. One case report documents a woman whose thyroid function flipped between hyper- and hypothyroid solely based on the balance of stimulating versus blocking antibodies [18]. Another patient transitioned from primary hypothyroidism straight into Graves' hyperthyroidism over a year [19]. The complication you need to watch for is thyroid eye disease (TED). A 2024 review notes that TED affects 25 to 50 percent of people with Graves' disease and can be sight-threatening, with new biologic drugs like teprotumumab now showing 83 percent of patients achieving significant proptosis reduction, though long-term safety data are still limited [38]. Pregnancy demands careful management, as maternal antibodies cross the placenta and can cause neonatal disease with a 12 to 16 percent mortality rate [16].
