What liver tests should I monitor on long-term tamoxifen?

The short version: the evidence you were given does not answer your question about which liver tests to monitor on tamoxifen.

The retrieved studies are almost entirely about hepatitis C, hepatitis B, NAFLD biomarkers, and drug-induced liver injury from other drugs (disulfiram, raltegravir, interferons). Only two records directly involve tamoxifen and liver tests. One rat study [18] measured AST, ALT, and ALP after tamoxifen and found elevations: AST rose from 128 to 203 IU/L, ALT from 51 to 62 IU/L. A 2025 rat study [20] confirmed tamoxifen raised ALT from 68 to 112 U/L, AST from 162 to 250 U/L, and ALP from 55 to 77 U/L. That is the entire human-relevant monitoring evidence in this retrieval.

No human trial in this set compared different monitoring strategies. No study tested whether checking ALT alone versus ALT plus AST plus ALP plus GGT catches injury earlier. No study examined how often to test (every 3 months, every 6 months). The 2015 human study [17] measured ALT and ALP in 47 breast cancer patients but was cross-sectional and did not compare monitoring schedules.

What standard oncology practice does, outside this retrieval, is check ALT, AST, and ALP at baseline and every 3 to 6 months. The retrieved evidence does not confirm or challenge that schedule. It simply does not contain the comparison.

My call: the evidence here does not tell you which tests to run or how often. The standard panel (ALT, AST, ALP) is what the animal data measured, but no human monitoring trial was retrieved. Confidence: not clear.
