(methylphenidate)
Methylphenidate for long COVID brain fog: one tiny case series says maybe, and nothing stronger exists in this retrieval.
The only record that actually tests your question is a case series of six adults with long COVID cognitive dysfunction, treated with low-dose methylphenidate for years-long symptoms. Five of six improved on memory testing; one got worse; side effects included fatigue, disturbed sleep, and stomach upset [1]. That is six people, no control group, no blinding. It is a signal worth investigating, not evidence of benefit.
The rest of the retrieval is about ADHD, Parkinson's, stroke, and driving studies, not long COVID. It shows methylphenidate is a real drug with real effects: in ADHD trials it improves attention measurably versus placebo [3], and common harms are appetite loss, insomnia, irritability, and mild heart-rate and blood-pressure rises [2][4]. A 2026 systematic review calls short-term tolerability acceptable but says long-term outcomes and rare serious risks remain uncertain [4]. None of that was measured in long COVID patients, whose fatigue and post-exertional crashes may respond very differently to a stimulant than ADHD does.
What this retrieval cannot see: any randomized trial of methylphenidate in long COVID, any data on whether stimulants worsen post-exertional crashes, and any long-term safety data in this population. Those are the studies that would settle it.
My call: the evidence is not clear. Six uncontrolled cases plus an ADHD safety profile is too thin to recommend and too thin to rule out; if you try it, it should be a cautious, doctor-supervised trial with a clear stop rule. Confidence: low.
Retrieval gap: no controlled long COVID trial surfaced; only the six-patient case series [1] matches the question.
