How often should B12 levels be checked in people taking metformin?
The short version: the evidence supports annual B12 screening for anyone on metformin, starting one year after initiation, with more frequent checks for those on higher doses or longer durations.
The evidence you already have establishes the problem clearly. Metformin causes B12 deficiency in a dose-dependent and time-dependent way, with rates from 6.3% to 35% depending on the study and definition [1]. The 2024 meta-analysis explicitly says the prevalence is greater than what the regulatory label declares [1]. A 2014 meta-analysis of 29 studies covering 8,089 patients found an odds ratio of 2.45 for B12 deficiency with metformin [3]. The 2026 Ugandan study found that a dose of 2000 mg/day carried an adjusted odds ratio of 4.63 for deficiency, and use for 10 years or more carried an aOR of 4.04 [4]. The 2016 Indian study showed mean B12 dropping from 183 pg/mL in those taking under 1500 mg to 118 pg/mL in those taking 1500 mg or more [5].
The question is how often to check. The evidence does not contain a single randomized trial that directly tests different screening intervals. No study has compared annual versus biennial versus symptom-triggered screening for hard outcomes like neuropathy, cognitive decline, or anemia. What the evidence does give you is the time course of the deficiency. The 2024 meta-analysis found that metformin exposure durations from 6 weeks to 48 months were all associated with decreased B12 concentrations [1]. The 2014 meta-analysis included studies with treatment durations from 6 weeks to 208 weeks (4 years) [2]. The 2026 Ugandan study found that deficiency risk jumped sharply at 10 years of use [4]. The 2016 Indian study showed B12 levels dropping from 163 pg/mL in those using metformin under 5 years to 101 pg/mL in those using it 5 years or longer [5].
The practical takeaway from this time course is that deficiency can develop within the first year and continues to accumulate over years. The American Diabetes Association standards of care recommend checking B12 levels annually in metformin users, and that is a reasonable minimum. The evidence supports starting the first check at one year after initiation, then annually. For patients on higher doses (1500 mg/day or more), those with neuropathy symptoms, or those who have been on metformin for 5 years or more, checking every 6 months is more prudent. The 2026 Ugandan study found that 35.7% of patients on 2000 mg/day were deficient [4]. The 2016 Indian study showed that B12 levels in the 5-year-plus group were already in the deficient range at a mean of 101 pg/mL [5].
The good news is that when deficiency is found, treatment is straightforward. A 2025 randomized study found that oral methylcobalamin 1000 mcg daily was as effective as intramuscular injections for restoring B12 levels [6]. So the monitoring question is not about whether you can treat it, but about catching it before it causes irreversible nerve damage.
My call: check B12 annually starting one year after metformin initiation, and every 6 months for those on 1500 mg/day or more, or after 5 years of use. Confidence: moderate. The evidence for the link is strong, but no trial has directly compared screening intervals to prove that annual is the optimal frequency.
