The short version: Ozempic was tested for surrogate endpoints like HbA1c and weight loss, not for whether it makes you live longer or healthier overall.

The SUSTAIN and PIONEER trials were funded by Novo Nordisk, the manufacturer [1, 13]. They compared semaglutide to placebo or other drugs in people with type 2 diabetes, showing it lowers blood sugar and body weight [1, 3, 11]. The largest cardiovascular safety trial, SUSTAIN 6, enrolled 3,297 high-risk patients and found fewer heart attacks and strokes (hazard ratio 0.74) but also more diabetic retinopathy events (3.0% vs 1.8%) [14]. That trial lasted only two years, and the retinopathy signal was never fully explained [14].

The weight-loss trials (STEP program) were also manufacturer-funded and lasted 68 weeks at most [6, 15]. They show people lose about 15% of body weight on the 2.4 mg dose, but gastrointestinal side effects are very common: nausea in 42% of patients in one NASH trial [2], and about 7% of people stop the drug because of side effects [11]. No trial has followed people for more than a few years, so long-term effects on cancer, pancreatitis, or thyroid disease are unknown. The evidence says the drug changes lab values and body weight in the short term. It does not say what happens to you five or ten years later.

My call: Ozempic was tested for short-term metabolic effects in manufacturer-funded trials, but the evidence for long-term safety and net health benefit is thin. Confidence: moderate for short-term efficacy, low for long-term safety.
