The short version: the evidence here does not test your hypothesis. It tests treatments for subacromial impingement, not whether you have it.

The studies you were given all assume the diagnosis is already made. They compare surgery to physiotherapy [12], laser to ultrasound [13], or stretching to nothing [7]. None of them answer the question "what is causing this pain?" The diagnostic test study [9] shows that the common physical exam maneuvers (Hawkins-Kennedy, painful arc) have poor accuracy—sensitivity 74% at best, specificity as low as 50%. That means a positive test tells you very little. The scapular dyskinesis test [10] also fails to predict outcomes. So the standard clinical tools are weak.

What the evidence does show is that conservative treatments—physiotherapy, stretching, manual therapy—produce meaningful improvements in pain and function over several months [4, 7, 12]. Surgery offers no advantage over physiotherapy at 2-3 years [12]. The omega-3 trial [2] found no added benefit over placebo. The mechanism that would tie this to colloidal science (impaired zeta potential causing microvascular sludging in the rotator cuff tendons) is plausible but not tested in any of these records. No study here looked at blood flow, red cell aggregation, or glycocalyx integrity.

My call: the evidence does not support any specific diagnosis for bilateral arm pain on elevation. The most studied treatments (physiotherapy, stretching) are low-risk and show real benefit, but the diagnostic tests are unreliable. Confidence: moderate that conservative care helps symptoms; low that the cause is identified.
