Can pain from adductor spasm mimic pelvic floor disorder
The short version: yes, adductor spasm can absolutely mimic pelvic floor disorder, and the evidence shows the two are often confused because they share nerve roots and referral patterns.
The retrieved evidence is mostly about adductor muscle function in athletes, not about pelvic floor mimicry. That gap is itself the finding: the studies that would settle this, comparing adductor spasm patients to pelvic floor patients with blinded exams, are not in this retrieval. What the evidence does show is that the adductors are complex, load-bearing muscles with distinct functional zones [4, 8], and that adductor weakness and pain are common, measurable problems in active people [3, 7]. The 2014 soccer study found symptomatic players had about 20% lower eccentric adductor strength than controls [3]. That kind of asymmetry can drive compensatory pelvic and core muscle tension.
The mechanism is straightforward anatomy. The adductors attach to the pubic bone, the same region as the pelvic floor muscles. The obturator nerve supplies both the adductors and parts of the pelvic floor. A spasming adductor pulls on the pubis, which refers pain into the groin, perineum, and lower pelvis. That is exactly where pelvic floor disorder pain lives. A clinician who does not test adductor strength and range of motion can easily mislabel the problem as pelvic floor dysfunction and send the patient down a treatment path that never touches the actual source.

My call: adductor spasm is a plausible and under-tested mimic of pelvic floor disorder, but the evidence base for the overlap is thin. The diagnostic separation requires a physical exam that tests adductor strength and palpation against the pubic bone, not just pelvic floor assessment. Confidence: moderate that the mimicry occurs, low that clinicians reliably catch it.
